Paid Time Off (PTO)
UnityPoint Health PTO plans across the organization are a simple, easy to use plan. All benefit eligible team members accrue PTO into one account that will combine vacation, individual sick days, holidays and personal days in order to give you maximum flexibility in scheduling time away from work. Providers should refer to their employment agreement for details surrounding their time off.
Note: Dubuque Team Members should refer to their local policies or the available benefit guides.

* Maximum accrual is based on the first 80 hours paid per pay period and 2,080 hours paid annually.
** To calculate your PTO accrual per pay period, multiply the “PTO Accrued per Hour” number above by the number of hours you are paid per pay period. For example, if you have 16 years of service and are a .75 FTE or are paid 60 hours per pay period, your calculation would be as follows: .1346 x 60 = 8.07 accrued hours in a pay period.
*** The six recognized holidays are New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.
Employee Assistance Program (EAP)
EAP provides team members with support to manage the stress and challenges of life including counseling sessions, health & wellness discount programs, local support services and other resources. The program is available to all UnityPoint Health team members without enrollment, and there is no cost to you. For more information, please call the EAP office in your region.

Test Your Mood
Test Your Mood is an online screening tool powered by Mindwise that offers 14 unique, confidential questionnaires to help you determine how you’re feeling. It’s an easy checkup that takes only minutes to complete and can be answered in the privacy of your home and on your own personal device – including your cellphone. Upon completing the anonymous survey, you’ll be presented with results, recommendations on next steps, and information on key resources. Most importantly, you’ll be empowered with the information you need to take the next steps in your journey toward better emotional and mental health.

AbsenceOne:
All approved Leaves of Absence (LOA), including intermittent Family and Medical Leave Act (FMLA) and continuous LOA, including Short-Term Disability (STD), medical, personal, and military leave, are handled by our third-party administrator, AbsenceOne.
Exceptions to this include:
- Insured short-term disability claims opened prior to September 1, 2021, will remain with Prudential.

Paid Parental Leave (PPL) Program:
UnityPoint Health believes it is important to support team members, both women and men, as they balance their work with the wonderful but challenging role of becoming parents, or as their families grow. Therefore, all full-time benefit eligible team members receive one week of 100% paid parental leave when they serve as a primary caregiver following the birth or adoption of a child. This benefit provides broad coverage that includes either the mother or father and both natural birth and adoption.
Bereavement Leave:
UnityPoint Health has aligned to a single Bereavement Leave policy across the system. In the event that a benefit-eligible team member experiences the death of a family member, UnityPoint Health will provide between one and five days off with pay to grieve, make arrangements due to the family member’s death, or attend the funeral. The amount of time provided is determined by the relationship to the family member.
Jury Duty:
UnityPoint Health has also aligned to a single Jury Duty policy across the system. In the event that a benefit-eligible team member is selected for jury duty, UnityPoint Health will allow team members to receive their regular rate of pay for scheduled time lost. Team members can also retain any money received from the courts for serving as a juror or a witness.

We support our team members overall health and well-being. Our wellness program, OnPoint for Health, incorporates overall well-being including physical, emotional, financial and social.

2025 Wellness Credit
It is a $20 per pay period credit added to your paycheck, rewarding you for making wellness a priority. The requirements, completed by the team member and a covered spouse/domestic partner, are as follows:
- Complete an annual physical with your primary care provider (PCP) between December 1, 2023 and November 30, 2024.
- Log into the OnPoint for Health portal between October 1st and December 20th to complete your online Health Risk Assessment (HRA).
- Please note, your spouse/domestic partner will also need to complete this step in order to be eligible for the 2025 Wellness Credit.
2024 Wellness Rewards
Benefit-eligible team members and their spouse/domestic partner enrolled in the health plan can earn points by completing challenges that will be available after January 1st, 2024 in order to earn financial rewards.
- Achieve 1,500 points and earn a $100 reward paid out on your paycheck
- Achieve 3,000 points and earn a $150 reward paid out on your paycheck.
*Eligibility for these rewards will be reviewed on a monthly basis between February and December 2024. The financial reward will be paid out by the 2nd paycheck of the following month in which the point total is achieved.
Please note: all wellness rewards are subject to tax and you must be in an active, benefit-eligible status at the time the reward is paid out in order to receive it.
Wellbeats & LifeSpeak
In 2021, UnityPoint Health launched a new health and well-being benefit called Wellbeats. Wellbeats is an easy-to-use fitness platform to help you feel happier, healthier and stronger.
Through Wellbeats, team members have access to over 1,200 classes, including yoga, HIIT, office-friendly workouts, strength training, mindfulness, nutrition, and so much more! Also available are 60+ goal-based programs available anytime and anywhere. In 2024, we’ve expanded our partnership with Wellbeats to offer LifeSpeak courses and programs. LifeSpeak courses provide confidential around-the-clock access to world-class experts in a variety of topics, such as stress and anxiety, mindfulness, relationships, parenting, sleep and movement, self-care, and PTSD.
