Wellness Rewards Program Under this Wellness Rewards Program, you may be eligible to earn rewards for completing certain health care and wellness activities. Wellness Rewards Program Eligibility When you are the enrolled employee and you complete certain health care and wellness activities, you will be eligible to receive wellness rewards. The Wellness Rewards Program operates on a point system. Points may be earned by employees who are age 18 or older when they complete qualified health care and wellness activities. The points you earn go towards wellness dollars that can be redeemed as described below. Employees have the opportunity to earn up to a maximum of $400 in each plan year, not to exceed $100 per plan quarter. In order to participate in the Wellness Rewards Program, employees are required to register. You can register by logging on to the following Web site at www.ahealthymerewards.com. Or, you can call the dedicated customer support telephone number Monday through Friday from 8:00 a.m. to 9:00 p.m. (Eastern Time) at 1-844-854-7285. Upon registration, employees can also order a free, daily activity tracker device. How to Earn Points You may earn points by completing certain health care and wellness activities. These activities include (but are not limited to):  Completing an annual personal health assessment.  Getting recommended routine exams, preventive screenings, and immunizations, including an annual flu shot.  Completing a quarterly physical activity challenge.  Achieving monthly milestones such as completing 10,000 steps within 20 days per month or tracking sleep for 20 days during a month.  Achieving daily steps or activity minutes.  Tracking daily healthy habits. Points and Rewards Through this program, the points you earn go towards dollars that can be redeemed for gift cards, merchandise, or deposits made directly into your personal bank account. These wellness rewards may be considered taxable income. Important Note: Your Blue Cross Blue Shield of Massachusetts health plan does not include health benefits for costs related to wellness rewards programs. This separate Wellness Rewards Program offers incentives for participation in qualified health care and wellness activities.
