Education Assistance Bene昀椀ts This bene昀椀t is an investment in your higher education and development. Full-time and weekend package plus bene昀椀t eligible team members may receive up to $2,000 per semester or course session, up to a maximum of $4,000 per calendar year. Part-time and weekend package bene昀椀t eligible team members may receive up to $1,000 per semester or course session, up to a maximum of $2,000 per calendar year. To be eligible for reimbursement the team member must complete an online application process prior to starting a course, and only for courses starting after the team member has completed six months of employment. Applications received after the class start date will not be accepted for reimbursement. Request for reimbursement and documents must be received no later than 90 days after successful completion of the previously approved course(s). Terms of Tuition Reimbursement: › Team member must obtain a “C” or better for Undergraduate degree courses › Team member must obtain a “B” or better for Graduate degree courses › The program will offer reimbursement for expenses relative to books, fees and tuition less any grants/scholarships. › Tuition r eimbursement bene昀椀ts do not include payments for the following items: • Meals, lodging, or transportation • Tools or supplies (other than textbooks) that you can keep after completing the course of instruction. • Courses involving sports, games, or hobbies unless they: – Have a reasonable relationship to the business of UnityPoint Health; or – Are required as part of an approved degree program. Public Service Loan Forgiveness (PSLF) Support PSLF is a federal program created for those in public service jobs, offering the opportunity to have your federal loan balances forgiven after 10 years and 120 qualifying payments. All tax free!
