Team members are eligible after successful completion of 90 consecutive days of employment. For reimbursement, proof of qualifying expenses must be provided within 60 calendar days of the 昀椀nalization of adoption. If spouses both work for UnityPoint Health, the reimbursement will be paid to the team member eligible for the higher amount.
