ELIGIBILITY Offering a comprehensive and competitive benefits package is one way we recognize your contribution to the success of the organization and our role in helping you and your family to be healthy, feel secure and maintain work/life balance. This enrollment guide has been designed to provide you with information about the benefit choices available to you. Remember, if you are a new hire, this is your only opportunity to enroll in coverage until open enrollment, unless you or your family members experience an eligible “change in status.” How to Enroll in the Plans • Read available materials and make sure you understand all the options available. • Complete your enrollment in the Employee Portal (www.workforcenow.adp.com). • Submit your benefits elections within 30 days of your date of hire. • If you have any questions or concerns, please contact your People & Culture Department. Whom Can You Add to Your Plan? Eligible: • Legally married spouse • Natural or adopted children up to age 26, regardless of student and marital status • Children under your legal guardianship • Stepchildren • Children under a qualified medical child support order • Disabled children 19 years or older • Children placed in your physical custody for adoption • Domestic Partners Ineligible: • Divorced or legally separated spouse** • Common law spouse, even if recognized by your state • Domestic partners, unless your employer states otherwise • Foster children • Sisters, brothers, parents or in-laws, grandchildren, etc. Change in Status Generally, you may enroll in the plan, or make changes to your benefits, when you are first eligible. However, you can make changes/enroll during the plan year if you experience a change in status. As with a new enrollee, you must submit your paperwork within 30 days of the change, or you will be considered a late enrollee. Examples of Change in Status: • You get married, divorced or legally separated • You have a baby or adopt a child • You or your spouse takes an unpaid leave of absence • You or your spouse has a change in employment status • Your spouse dies • You become eligible for or lose Medicaid coverage • Significant increase or decrease in plan benefits or cost ** unless stated in divorce decree

Please review your plan summaries or SBCs for out- of- network coverage information and full plan details. Click on the plan name to access plan summaries. Will you be covering family members on your medical plan? If so, each of your family members would also be subject to the individual deductible, but if the family accumulates to the family deductible, no further deductible is required by any family member. Likewise, each of your family members would also be covered 100% if they reach the individual out-of-pocket maximum, but if the family accumulates to the family out-of-pocket maximum, no further family members are subject to expenses.
FLEXIBLE SPENDING ACCOUNT (FSA) FSAs offer opportunities to pay for eligible out-of-pocket health care expenses and dependent care expenses with pre-tax dollars. You have the option to participate in the following FSAs: FULL-PURPOSE FSA Who can participate? Employees not enrolled in a medical plan, or employees enrolled in the company OAP medical plan. What happens at the end of the year? The Plan provides for a 2 ½ month grace period immediately following the end of each Plan Year. Contribution Limits: Employees can contribute up to $3,050 per plan year. You can use the funds for any qualified healthcare expense DEPENDENT CARE FSA Who can participate? Any employee What happens at the end of the year? The Plan provides for a 2 ½ month grace period immediately following the end of each Plan Year. Contribution Limits: Employees can contribute up to $5,000 per plan year. You can use the funds for any qualified dependent care expenses.
LIFE AND DISABILITY LIFE INSURANCE Basic Life and Voluntary Life insurance provides for financial support in the untimely passing of a covered participant. Click on the blue highlighted plans below for a plan summary. Basic Life & AD&D • Benefit amount: 1.5 times annual earnings up to a maximum benefit of $225,000. • Please be advised that should you reach age 65, your coverage will reduce by 35%. Should you reach age 70, your coverage will reduce by 50%. • Employer Paid Voluntary Life • Employees can elect Voluntary Life and AD&D for the employee, spouse, and dependent children. • The cost is based on the amount you purchase and your age at the time of purchase • Employee Paid Note: If electing outside of new hire period, Evidence of lnsurability will need to be submitted Disability Short and Long-Term Disability insurance provides partial income replacement in the event of a covered illness or accident that occurs outside of work. Short-Term Disability •Benefits kick in once you have been out of the office due to your disability for 7 days or illness/0 days for accident •Weekly benefit checks will be 60% of your weekly earnings to a maximum of $1,500 a week •You can receive these weekly benefits checks up to 13 weeks for sickness/13 weeks for accident, if you continue to be out of work due to your disability • Employer paid Long-Term Disability •Benefits kick in once you have been out of work for 90 days •Monthly benefit checks will be 60% of your monthly earnings to a maximum of $12,500/month The benefit plan information shown in this guide is illustrative only. COLONIAL LIFE VOLUNTARY BENEFIT OFFERINGS FROM COLONIAL LIFE All Acme Corp benefit eligible employees have the opportunity to enroll in Voluntary Benefits from Colonial Life. With voluntary products from Colonial: • Benefits can be customized to fit your individual needs • Plans provide coverage that fills the gaps of your employer paid benefits • Coverages are available for both you and your family • You will enjoy the convenience of premium payment through Payroll Deduction. Accident Insurance: Helps offset the direct and indirect expenses such as deductibles, co-payments and other costs not covered by traditional health plans. Spouse and dependent coverage available Critical Illness Insurance: Provides benefits to help pay for medical and non-medical expenses related to cancer that most health plans do not cover. 401K - QUALIFIED DEFAULT INVESTMENT ALTERNATIVE To help you prepare for retirement, ACME Corp Inc offers you a retirement plan. You can choose to invest the contributions made to your account in a number of investment options. This notice describes the default investment feature applicable to the plan. Joining the Plan If you haven’t already, you can enroll in the plan by taking the following action: •Elect the plan in ADP under the Year-Round Enrollment option (Myself- Benefits- Enrollments) •Your election(s) will become effective as soon as administratively feasible after receipt and processing of your election. Details of Your Default Investment Unless you choose or have already chosen another investment, your contributions will, or will continue to be, invested in the default option for the plan. The default option for our plan is the American Funds Target Date Retirement Series ® Unless you’ve been notified otherwise, the default fund is the fund in the series that is available at the time you are first defaulted that most closely matches the year you will turn 65 (see the chart below). What If I Do Not Want My Contributions Invested in The Default Fund? If you haven’t already made your investment selections, you can avoid having your contributions invested in the default fund by taking the following action within 30 days from the date of this notice. Access your account by visiting the plan’s website at myretirement.americanfunds.com or by calling the toll-free number at (800) 204-3731 and make your investment selection. Changing How Your Investments Are Being Invested You can change your investment allocation at any time, or you can make an exchange from the default investment into any other investment available for our plan by going to the website myretirement.americanfunds.com or calling the toll-free number, (800) 204-3731. There is no transaction fee for making an exchange into one of the other investment options available in our plan. Description of the Default Investment Designed to simplify your investment decision-making, the American Funds Target Date Retirement Series is made up of 12 target date fund portfolios, each composed of a different mix of the American Funds, with retirement dates ranging from 2010 through 2065 in five-year increments. New funds may be added to the series for future retirement dates as needed. Each target date fund serves as a single diversified retirement portfolio — with an underlying investment approach aligned with its retirement date — so you only need to select one. Each fund in the Series attempts to balance investors’ long-term needs for both return and conservation of capital.
